Your health and wellness practitioner is your primary support on this health and happiness journey. Their primary responsibility is creating an environment focused on developing self-awareness, compassion and self-confidence to enable you to focus on you!

Some other responsibilities of your health and wellness practitioner are:

  • on the same journey and understands the challenges of lifestyle change
  • provides a safe, professional, non-judgemental support
  • only recommends lifestyle changes on evidence based research
  • guides you to reflect on your beliefs, thoughts, feelings and actions and how they impact on your health and happiness

Your health and wellness practitioner is not a substitute for a medical professional. They do not provide medical advice, diagnosis or treatment. Yield encourages you to involve your medical professional in any appointments, especially if you have any long-term or chronic health conditions.

Arrange a Session Time

To arrange a session with a practitioner you can either arrange online or message Yield to make a time. Feel free to ask any questions you may have about the journey so that you can feel confident in making your next step.

Platform

All of our programmes are managed on a platform supported by Better Practice. This allows you to have a safe, secure way to access your programme on whichever device is most convenient for you. Once you have arranged a session time you will receive confirmation of your session and be encouraged to register for the platform to make communication easier. 

Purpose Session (15min)

Once you have registered you will receive a welcome email outlining the health and happiness journey with Yield. Before your session you will receive a health and happiness questionnaire which provides you with the opportunity to reflect on where you are now and create clarity around your purpose for change in preparation for your purpose session.. 

Your initial 15min purpose session will involve discussing the health and happiness journey and clarifying any questions you may have about questionnaire, the platform or the programme. This is the perfect opportunity to bring up any concerns you may have or aspects of the programme that you would like to have explained in greater detail. Then you can schedule your 60min planning session with your health and wellness practitioner. 

Planning Session (60min)

As part of the planning session you will have a comprehensive discussion around your current health and wellbeing based on your responses to the health and happiness questionnaire. This will clarify a clear purpose co-created by you and your practitioner, helping you discover more about who you are, what you want and creative ways to move forward. You will receive personalised support and practical solutions to identify achievable changes to assist you with optimising your health and well-being.

After each session your health and wellness practitioner will provide a self-care plan and personal protocol via the platform to guide your steps before your next session. Your programme will be available on the app for you to review daily. With food, mood and lifestyle journals available to help you measure your progress points.

Potential Session (30min)

As part of your planning session you will have the opportunity to discuss how often you would like to have reviews of your progress. We recommend having at least monthly reviews with your health and wellness practitioner to receive personalised guidance and support. Some of our clients prefer weekly reviews for changes that require more encouragement and accountability. It’s completely up to you as to how much support you feel you need.

Yield offers our services as 12-week programmes. We’ve found this provides the most supportive experience and opportunity for meaningful lifestyle change. All of the programmes come with the guidance of your own health and wellness practitioner with varying levels of support depending on what you need on your journey. You can also design your own programme based on a combination of the session types below, or if you would like to add to your self-directed programme at any time just get in touch and we will tailor a programme to meet your needs.

Planning Session (60min) – Access + Planning Only ($89)

If you would just like a plan review either due to seasonal or life changes you can arrange an independent planning session as you need. This is only recommended if you have already had a purpose session to ensure that the health and wellness practitioner is aware of your current health and happiness aspirations. 

This includes:

  • self-care plan
  • personalised protocol 
  • online access to your programme through our platform
  • online journal to track your progress points
  • secure online chat with your health and wellness practitioner 
  • subscription to our quarterly Yield magazine (online only)
 

Potential Sessions (30min) – Access + Review Only ($49)

We recommend having at least monthly reviews of your progress with your health and wellness practitioner to receive personalised guidance and support. So it’s recommended that you have had a purpose and planning session prior to arranging your potential session. Some of our clients prefer weekly reviews for changes that require more encouragement and accountability. 

This includes:

  • weekly reviews of journal entries by your health and wellness practitioner 
  • comprehensive discussion of your progress points and journal entries with guidance from your health and wellness practitioner.
  • review of your self-care plan 
  • evaluation and update of your personalised protocol as required

As part of your initial purpose session you have the flexibility to choose how often you would like to review your self-care plan and the level of support and encouragement you need. You can either arrange in advance to secure a time that works for you, or arrange sessions as you need. 

The choice is yours! Want to focus on another area of your health and wellness . . . no problem. Simply enrol for another 12 week programme. There is 10% discount if you choose to enrol for another programme.
